Registration Materials for Fall Gathering
WQM Oct. 3-5, 2008

Below you will find links to information sheets and forms which you may use to register for the Fall 2008 WQM gathering.
Please note that you can register in one of two ways.
- If you want to print the materials, fill in the forms by hand and send them by mail:
We suggest that you use these pdf files. To use these files you must have a program like Adobe Reader on your computer that reads Adobe Acrobat files. Mail the completed form to the address at the bottom of the registration form. (You can could also print word document files under option 2. below and fill them out by hand.)

- If you would rather use a word processor to fill in the form, you can send the completed form back as a file attached to an e-mail message:
To register by e-mail, download the file and fill in the information using your word processor.
Save the file.
Then send the file(s) as an attachment to an e-mail message to the registrar at the e-mail address listed at the bottom of the registration form.
Microsoft word and many other word processors can read this type of file.
